An OAE hearing test is a quick, painless check of how well your inner ear responds to sound. It takes only a few minutes per ear and needs no response from the person being tested. That makes it especially useful for sleeping newborns.
In Malaysia, the OAE hearing test is the first checkpoint in the newborn hearing screening programme. Adults encounter it too, usually as one part of a full hearing workup. In this guide, we will explain what an OAE test is, how it works, and where it fits among types of hearing test.
What Is an OAE Hearing Test?
An OAE hearing test measures otoacoustic emissions, which are faint sounds that healthy cochlear hair cells produce in response to a stimulus. Think of them as tiny echoes from the inner ear. When the echoes are present, the cochlea is responding as expected.
This test checks inner ear function, so it works best as a screening step. It shows whether the echoes are there, but it cannot measure the degree of hearing loss. Other tests provide that detail.
Its main strengths are speed and comfort. Because it needs no response, it suits babies who cannot yet tell us what they hear.
How Does an OAE Hearing Test Work?
An OAE hearing test works by placing a soft probe in the ear canal and listening for the cochlea’s echo. The probe contains a tiny speaker and a microphone.
Here is what happens during a typical test.
- Clicks or tones: The probe plays a series of soft sounds into the ear canal.
- Echo response: Healthy hair cells reply with a faint echo.
- Recording: The microphone captures that echo, usually within a couple of minutes per ear.
If the cochlea, outer ear, or middle ear is blocked or not working properly, little or no echo returns. In that case, the test flags the ear for follow-up and does not give a diagnosis.
OAE Testing for Newborns in Malaysia
OAE anchors Malaysia’s Universal Newborn Hearing Screening programme because it is fast, painless, and needs no response from the baby. Most babies are screened before leaving the hospital. If the first result is inconclusive, a repeat screen follows. Any baby who does not clear it goes on to a confirmatory auditory brainstem response (ABR) test.
An initial “refer” result is common. Residual fluid in a newborn’s ear canal can affect the reading, which is exactly why the retest step exists. As a result, a refer result simply signals that a second look is needed.
Types of Hearing Tests Beyond OAE
No single test tells the full story, so audiologist will conduct comprehensive hearing evaluation to get the full picture of the hearing system, such as
- Tympanometry: Checks how the eardrum moves.
- Auditory brainstem response: Assesses hearing in infants or people who cannot respond behaviourally.
- Speech testing: Shows how clearly you understand words.
Alongside these, the pure tone audiometry test remains the gold standard for measuring the degree and pattern of hearing loss across different pitches. OAE alone cannot provide that detail.
When Might Adults Need an OAE Test?
Adults can benefit from OAE testing too. It helps assess noise-induced damage, monitor the effects of certain medications, and support diagnosis when a patient cannot respond reliably to standard testing.
As with newborns, a referred or unclear OAE result in an adult simply leads to further testing. If you are due for a hearing check, ask your audiologist which combination of tests suits your situation. An OAE test is a useful first checkpoint, and a complete picture of hearing health comes from combining it with other assessments. At The Listening Lab, our audiologists guide both newborns and adults through the right sequence of tests, with clear explanations at every step.
Where results point to hearing loss, a professional hearing aid fitting in Malaysia ensures the right solution follows a confirmed diagnosis. Read our guide to find out what to expect from the first 30 days after a hearing aid fitting.
